Your redemption table may have wrong column name. To fix this run MySQL query, on phpMyAdmin for example:

ALTER TABLE `redemptions` CHANGE `upgrade` `upgrade_acct` CHAR(16)

Note: If you use prefix on your MySQL tables, add it before redemptions.
